Sample Submission Forms

For submitting insect samples for identification

Knowing your pest is critical to good integrated pest management.  If you have an insect you would like to ID, you can send a sample to our office for a closer look.  Use one of the following forms:

> Large Insect Submission Form

if you can see discrete insects and can collect them without crushing them.

> Small Insect Submission Form

if you can see them, but will most likely kill them in the process of removing them from plant material (due to small size).

> Spotted Wing Drosophila Submission Form

if you are sending samples specifically for spotted wing drosophila identification, use this form.
